Albany Area Primary Health Care Encourages Patients to be their Own Health Care Advocate on Health Empowerment Day

Staff Report From Georgia CEO

Friday, September 18th, 2026

Wednesday, September 9, 2026, marks Health Empowerment Day, a
national awareness day focused on helping health care patients become more confident, informed, and engaged in their health care decisions. The initiative encourages individuals to be their own health care advocate - taking an active role in managing their health by learning about preventive care, understanding treatment options, utilizing available health care
resources, and asking questions that support better health outcomes.

“At Albany Area Primary Health Care (AAPHC), we believe that empowered patients are healthier patients,” said Dr. Edward Vance, Chief Medical Officer. “When individuals have the knowledge, tools, and support they need to make informed healthcare decisions, they are better equipped to manage, prevent, and improve their overall well-being.”

Health empowerment is about giving patients the confidence to participate in their health care journey. It means understanding health needs, communicating openly with health care providers, and taking advantage of resources that help you make informed decisions about your care. Every step toward greater understanding can lead to better health.

“At AAPHC, our dedicated health care teams work alongside patients to provide education, guidance, and compassionate care every step of the way. We believe that health care works best when patients and providers partner together to achieve healthy outcomes,” said Vance.

Ways patients can take control of their health care include:

• Scheduling annual checkup or wellness visit. AAPHC has clinics across Southwest Georgia that offer a variety of services that may include - adult & pediatric primary care, dentistry, podiatry, women’s health, pharmacy, vision, and behavioral health services.

• Stay current on recommended screenings and immunizations. Preventive
screenings and vaccinations are important tools for protecting your health and detecting potential issues early.

• Review medications and health concerns. Keep an updated list of medications, write down any symptoms you are experiencing, and bring questions or concerns to your provider. Open and timely communication with your provider helps ensure you receive the most effective care possible.

• Access and utilize online patient tools and resources. Technology can make
managing your health easier. Your AAPHC Patient Portal, located on aaphc.org, provide patients with access to their medical records and a way to communicate with their AAPHC health care team.

• Set achievable health goals related to nutrition, physical activity, sleep, or stress management. AAPHC patients who are looking to build a foundation of a healthy lifestyle, ask your provider about our FreshRx program. It’s a holistic approach to your well-being! Participants will receive nutrition education, fresh produce, ideas on how to prepare food, wellness support, lifestyle medicine guidelines, and more.

• Ask questions during appointments to ensure you fully understand your care plan. Don’t hesitate to speak up during appointments. Understanding diagnosis, treatment options, medications, and care plan can help you make confident health care decisions.

• Learn about available community resources that can support your health and wellness journey. Many community programs and support services can help individuals and families improve their health and well-being. Taking advantage of these resources can provide additional support on your health journey.

Small actions today can make a significant difference in your health tomorrow.
