Phoebe has recently launched our 2025 Community Health Needs Assessment (CHNA) which will inform our community benefit strategic plans for the next three years. A major part of the CHNA is collecting survey feedback from residents within our service area. The feedback from the survey will be used to identify opportunities to partner with the community to address social drivers of health that negatively impact the communities we serve. This feedback will also be used to identify the top health priorities of residents of SW Georgia. 

The survey is open until June 23rd and is available for anyone who lives in any of the counties listed below:
