Albany Area Primary Health Care Named Hypertension Control Champion

Albany Area Primary Health Care recently became one of 18 entities nationwide, and one of two in Georgia, to be named a 2015 Hypertension Control Champion by the Million Hearts campaign.
The 18, which also includes Kaiser Permanente Georgia and The Southeast Permanente Medical Group in Atlanta, used evidence-based strategies and patient engagement to help their patients achieve blood pressure control rates at or above the Million Hearts target of 70 percent.
