The National Committee for Quality Assurance is pleased to announce Albany Internal Medicine's recognition as a Level-3 Patient Centered Medical Home. The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork and technology to deliver quality care and improve health”, said NCQA President Margaret E. O’Kane. “NCQA’s PCMH Recognition shows that Albany Internal Medicine has tools, systems and resources to provide their patients with the right care at the right time.” 

The patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want to be". Medical homes lead to higher quality and lower costs, and can improve patients', patients' families, and providers' experience of care. NCQA Patient-Centered Medical Home Recognition is the most widely-used way to transform primary care practices into medical homes.
