Horizons Community Solutions Expands Cancer Screening Efforts with Launch of “Navigation Plus”

Horizons Community Solutions announced the launch of Navigation Plus, an expanded and enhanced version of its long-standing Community Cancer Screening Program. The initiative builds on over 15 years of success connecting uninsured and underinsured Georgians to lifesaving cancer screenings—and now extends its reach across South Georgia with a new name, modern tools, and a patient-centered approach.

A Stronger Path Forward

Navigation Plus connects individuals to essential cancer screenings—such as mammograms, colonoscopies, and the newly added low-dose lung CT scans—with the support of a personal Patient Navigator who assists them from scheduling through follow-up care.

Key program enhancements include:

  • Expanded access for underserved residents across South Georgia

  • New navigation for lung cancer screening, with prostate and cervical cancer navigation in development

  • Stronger integration with local providers and care networks

  • A refreshed digital experience for easier access to resources

Proven Results and Community Impact

Since its original launch in 2008, Horizons’ navigation model has consistently increased early screening, follow-up rates, and engagement in care—helping close the gap in cancer outcomes for rural and underserved communities.
