Violations: Observed PHF(potentially Hazardous Food) items uncovered in walk in cooler ( ex. Hotdogs , white rice, Raw bacon). Observed PHF in containers without proper date markings and disposition times, in walk in cooler. Observed:Cracked & missing "coved" floor tiles at the base of floor in prep area and service window~ drive thru. Observed: Missing "coved" base plate at entrance of deep freezer walk in cooler. Observed: debris found on walls ( Internal compartment) walk in cooling unit ( refrigeration) , where (PHF's) are stored for cooling.

Violations: Observed rice, meatballs, crawfish, shredded carrots, fish, wonton, and shrimp stored uncovered in coolers and subject to contamination. Observed fume hood in soiled and in need of cleaning. Discussed routinely cleaning with PIC. Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit.
