Equipment and utensils not properly air-dried. Observed shelving in coolers soiled. Observed bathroom doors left open other than during cleaning or maintenance. Bathroom not enclosed with tight-fitting, self-closing doors. Observed floor in need of cleaning near baseboards, beneath shelving, in walk in cooler. Observed opening in wall around plumbing fixture at vegetable prep sink. Observed hole in ceiling, ceiling tiles in need of replacing/repair
